The Registration Documents that has to be filled up consists of the following forms:
1. Acknowledgement - 1pg
2. Glucklich Registration - 2pg
3. Consent - 1pg
4. Medical Record - 1pg
5. Authorised Individuals to Fetch Child (if any) - 1pg
6. Emergency Contacts - 1pg
7. Model Release - 1pg
8. MCYS Form 1 - 5pg
Supporting Documents needed from the Parents are as follow:
1. Birth Certificate of Child
2. Both Parents' NRIC (back and front)
3. Birth Record Particulars of Child
4. Immunization record from the Health Booklet
5. Latest Payslip, CPF Statement and/or Latest Employment Letter of Parents/Single Parent
Please ensure that ALL the documents stated above has been filled, signed and/or presented before handing over for further process.
When the Registration is complete, hand to the new Parents, the MCYS' "Guide for Parents with Children in Child Care Centres". Please ensure that the last page of the book is signed, and filed at the top of the child's registration file.
